Use these two links:

If iTunes doesn't recognize your iPhone, iPad, or iPod - Apple Support

Then for a disabled iPad follow this link EXACTLY:

If you forgot the passcode for your iPhone, iPad, or iPod touch, or your device is disabled - Apple Support
