After software upgrade the ITunes app was removed from my iphone. How do I get it back?

There isn't (and wasn't in the past) an iPhone app with the name "iTunes." There is an "iTunes Store" and iTunes Remote" and iTunes U" app.
There is also an "Apple Music" app.
