Am no expert in this area ... yet your post deserves some response.
If you haven't seen them already: Wondering if Apple support's online articles (links below) may be helpful:
Eg: "a Bluetooth signal can be interfered with or weakened in some situations".
See: If the sound from your wireless headphones cuts out or is distorted - Apple Support
The interference noise may be from the iPhone:
Eg: says: "If you can’t hear a person’s voice clearly, hear crackling or static, or are experiencing issues with sound quality, find out what to do."
You could also check that any openings in the hearing-aids are clear,
and that the microphone on the iPhone is not obstructed.
You may want to check for a possible hearing-aid firmware update, (or, if you use an app, an app update) with your hearing-specialist, hearing-aid manual, or with the hearing-aid manufacturer (Eg: contact "support" on their website).
