Any subscription music that you download will become unplayable if the subscription lapses.

Any music that you have prior to the subscription is normally unaffected, but many people report problems with this. Make a backup of your music prior to starting the subscription.

Yes! Any Playlists created will also be removed too. Streaming content is only playable while you have a subscription whether it has been downloaded to the device or not. Downloading content with the subscription is not the same as purchasing the songs like you can in the iTunes Store. You don't own the music downloaded with the subscription.
