Personal & Family Medical records template

I would like to set up a spreadsheet/table of family history of medical conditions as well as track my own medical history. I am not good with creating a template or using the numbers/pages to their full extent. I have a Macbook Pro and would like to use something compatible as I do not have access to Microsoft products. Is there something anyone uses to track this type of information?
