Highlights shift or misalign after reopening PDF in Preview

I’ve recently noticed an issue with the Preview app on macOS. When I use the highlight tool to mark text in a PDF, everything looks fine at first. However, after I close the file and reopen it later, the highlights sometimes appear misaligned — they shift slightly up or down and no longer match the text I originally marked.

This problem happened in 2 out of 3 PDF files I worked on. All the files were standard text-based PDFs (not scanned images). I’m wondering if anyone else has experienced this issue or found a fix. Could it be related to how Preview renders fonts or saves annotations?

Any suggestions or workarounds would be greatly appreciated.
