Your MBP seems to have taken its time completing that EtreCheck report - 7:05 - and you apparently have some unsigned files that could use some culling. You can use Activity Monitor to discover which process is eating your CPU cycles. Activity Monitor User Guide for Mac - Apple Support

Your drive write/read speeds look pretty good: Write speed: 2909 MB/s Read speed: 2951 MB/s

To my eye this report doesn't look bad. If I've missed something - I do that - we hope another user can point it out.

Can you tell us with some more specific detail what exactly is driving you crazy?

— Check for a stuck upload to iCloud. I see issues with both Mail and Contacts that may be causing this alert at te top of the report:

Runaway user process - A user process is using a large percentage of your CPU.

— See this outside article for info on another issue, photoanalysisd:

If this just started in the last three days, restart you computer. Tha might help with the backlog of iCloud issues.

May be the system extension that triggered the "orphan extension" flag. Check at System Settings > General > Logins & Extensions and delete if not needed.
