All saved passwords lost

All of my passwords for the web and apps have gone. Any idea how I can retrieve (all 400 of) them?! I’ve had several restarts and change of settings over the past few days due to a separate issue (now fixed), but I can’t attribute any one thing to all my passwords to be wiped out.
