I bought my s4 Watch in the US. I never activated my watch until now that I’m abroad. I can’t activate ECG it always complains about my region, even though the Watch was purchased in the US! Help what can I do?!

For the ECG feature to work you must be in the same country where that feature is authorized In other words the ECG feature will only work with a Carrier in the United States.
deggie is right. When it first came out some people were able to fool the system by changing the Region. Shortly after, Apple closed that loophole in one of their software updates.
Then go ahead and use it or ask one of the people who were able to get it to work in a different region to help you. But then again it can also be that Apple has plugged this loophole with the latest watchOS.
Afaik there's plenty of people who use this feature outside the US.
