I just got charged $9.99 for an unauthorized in app purchase for an app that hasn't been used in years. Apple support merely blew me off. How do I stop in app purchases from being automatically charged to prevent this crap going forward?

If you have subscriptions that you no longer want : View, change, or cancel your subscriptions - Apple Support
If you don't want in-app purchases to be possible you can block them via Settings > General > Restrictions
