Verify your Apps' Setting(s):

Have you installed any Apps recently? If so, verify there is no Third-Party App(s) whatsoever overruling the Volume Settings

Try Disabling Bluetooth:

What happens when you disable Bluetooth(do so using Settings)? Does that make the audio go up? Perhaps, some remote hardware or software is overruling the Volume Settings.
