Normally, it can't be, unless you gave someone your Apple ID and password. Why do you think you have been hacked?

As long as your iPhone passcode and Apple ID password are secure (and you use two factor authentication), it is nearly impossible.

(The only reason I say "nearly" is that you can't prove a negative. I think the FBI was able to, but only at great expense and effort which probably required disassembling the iPhone)
It is possible for an iPhone to be hacked.
