Buying new iPhone

I currently own a 6S plus iPhone and my carrier is At&T. I want to buy an iPhone 12 Pro Max but will remain with the current plan. Do I have to buy the new phone from my carrier or can I just order a new iPhone from the Apple Store ? More specifically will my new phone come connected to my carrier with my current plan ?
