Unlinking Linking and Changing Payment Method

I accidentally linked my daughter's iphone to my Apple ID. I cannot find that phone linked to my ID anywhere. I want to unlink it from my ID, link it to her ID, and then change the payment method for her AppleCare from my old debit card to a new one. HOW??

How do you know it is linked? On her phone, have you checked Settings/Your Name to see if your ID is logged in there?
