Apple Care+ for Mac does not offer a monthly option. Only Yearly. Which will continue until you cancel it or Apple decides it can no longer support or repair the Mac.

Apple Care+ for iPhone or iPad which does offer a monthly plan, does not offer 3 years, but 2. and yes the Monthly plan would continue until cancelled or Apple decides it can no longer support the device.

While it's true there's no monthly plan for Macs, the linked page above seems to indicate you can renew.
Never said you couldn't renew. You did.
If you have the Yearly Plan form the beginning it will just continue yearly past the 3 years. No renewal necessary.
If you buy the 1 time purchase option for 3 years, then yes, you can renew the plan to the Yearly option after 3 years.
Not according to the page linked above. You would have to sign up for extended coverage within 30 days as explained.
