If I already have apple care and buy a new iPhone do I need to buy apple care again?

Yes, but if the old plan hasn't expired you can get a refund for "not used" protection.

Yes. Apple Care is tied to the specific device it was purchased for, and is not transferrable to another device.

You must purchase Apple care for each device you want to have it.
