Has my iPhone been hacked?

I just got a notification that my phone had been restored from icloud back-up, but I did not perform a restore. Has my phone or iCloud been hacked?

The only way to restore any backup is to wipe the phone back to factory settings and set it up again from the backup.

iCloud cannot be hacked unless you divulged your password and then accepted a two factor auth code request

All in all the notification appears to be false.

Do you have a screen shot of it? (blank out any personal details)
