I have low power turned off low power mode in the battery setting. Everytime i charge my phone and undo the sleep mode the low power mode comes on even at 100% charging. I have a medical device that requires 24/7 access to send my heart information.

That should not be happening. Low Power Mode will come on automatically if the battery drops below 20%, but it should turn off once charged to 80%, unless your iPhone has a failing battery. Go to Settings/Battery/Battery Health.
At the top it should say Normal. If it doesn’t have the battery checked by Apple and probably replaced→iPhone Battery Replacement - Official Apple Support
