Am no expert in this area: Fortunatyely, Apple support provides helpful guidance via articles online.
Wondering if pairing is ok,
which settings that you want to use are not working for you,
and what happens when you try.
Also: There may be firmware in use that may need updating for compatibility.
(eg: The user-manual or manufacturer's support website may have details)
You may want to check whether your cochlear implants are on Apple's Mfi compatibility list:

Control your hearing device
When your hearing device is connected to it, your Apple device can control microphone level (volume), audio presets, and more for your hearing device. Find the settings here:

On iPhone and iPad, you can also turn on Live Listen from the settings screen.
