Is your iPhone updated to latest version of iOS, 18.3.1? Are you using a website or mobile app to add passes? Do you a solid WiFi and cellular connection?
Are you using the mobile app or website? I fly Delta occasionally and find their website more reliable than their app.
What is the exact error message you when you try to add a boarding pass?

trying to check in for Delta flight. Received my Delta boarding pass but wallet will not accept it? It normally does.
