Inappropriate content in Apple Music despite content restrictions

How was my son able to pull up highly inappropriate stuff in Apple Music with all the content restrictions in place? . Now, I want to know how I can make sure he doesn’t delete his “recently played.” Because one more time and he loses any music app.
