Is There a way to increase the sound level on iPhone beyond what is available at purchase?

He might want to check with his audiologist or the manufacturer of his aids. If the hearing aids are BlueTooth compatible, he may be able to connect them to his phone.
It's also important to remember that you can only raise the earpiece volume while you're on a call. So, your friend should make a call and then use the side buttons to raise the volume.
A BlueTooth speaker would not be appropriate or relevant in this situation.
