Does this only apply to users who have IOS versions lower than 10.3.4? My software version is showing 12.4.1 - so do I need to do the update for my iPhone 5?
You have an iPhone 5s if you are on iOS 12. You are fine.

No, and there is no 10.3.4 for your device. If you currently have 12.4.1 installed you likely have a 5s or SE. Only the iPhone 5 is affected by the GPS bug, but not newer phones like the 5c/5s/SE.
