Sorry but I don't know of any way to resolve the issue by filling out a complaint form.
I would use the instructions in If you see a message that says 'Your account has been disabled in the App Store and iTunes' - Apple Support
and that may not be easy as lately it frequently results in "We’re sorry, there are no solutions available at the moment. Please try again later."

I am dealing with the same issue
I can't even contact Apple support, when I go to the support page it says "We’re sorry, there are no solutions available at the moment. Please try again later." I have no clue what to do.. I don't know why this happened either, I can login to my Apple account perfectly fine as well.
