I have this issue where a black box will appear when a photo is taken with my iPhone 16 camera. Please help!

I've never see anything like that. If you do a Forced Restart and it doesn't go away, I'd agree with LD150 and believe you have a real issue with the camera on your iPhone and an appointment at your Apple Store Genius Bar would be in order.

Assuming the box rotates along with image when the phone is rotated to landscape, it looks like a serious fault in the camera sensor.
