Icon on all pages

I have a black square icon with a white circle. When I touch this icon it shuts off my screen. How can I

remove this icon?

You have managed to activate Assistive Touch. You can configure and/or disable Assistive Touch in iPad Settings; to disable:

Settings > Accessibility > Touch > AssistiveTouch > AssistiveTouch - set to OFF
